All point structures are once per year unless otherwise noted.

Primary Nurse coaching
The Primary Nurse Health Management Program provides assistance with severe and complex care needs, including cancer, high risk pregnancies/NICU care, transplants and infusion therapy.

Engage with a nurse four times in the year: 250 points

Horizon CareOnlineSM (telehealth)
DSM medical plan participants can contact the telehealth program for help with non-emergency medical issues, such as cold and flu symptoms, migraines, allergies, pinkeye and stomach pain. Doctors can diagnose and treat many medical issues, as well as write prescriptions.

Registration: 200 points
Visits (maximum of two per year): 100 points

Annual physical
Receive an annual physical exam from your doctor: 1,500 points

Biometric screening
Complete a biometric screening at a lab or a doctor’s office: 500 points

Colonoscopy
Get a preventive colonoscopy exam (if recommended for your age): 1,000 points

Dental exam
Receive a routine dental exam (includes but is not limited to regular cleanings): 1,000 points

Health assessment
Complete a health assessment to better understand your overall health: 1,000 points

Mammogram
Get a preventive mammography screening exam: 1,000 points

Pap test
Get a preventive Pap test from your doctor: 1,000 points

Prostate specific antigen (PSA) or digital rectal exam (DRE) prostate cancer screening
Get a PSA or DRE prostate cancer screening: 1,000 points

Vision exam
Receive an eye exam from your optometrist: 1,000 points

Wellness Corporate Solutions (WCS)
Virgin Pulse’s telephone-based coaching through Wellness Corporate Solutions provides tobacco cessation coaching in a convenient format. Engaging with a health coach provides the support and accountability needed to reach your goals, reduce your risk of disease, improve your health. Plus, when you participate, you can get your tobacco use surcharge waived. Morning, evening and weekend appointments are available. Connect with a coach at Virgin Pulse.

Completion of first call: 200 points
Completion of four tobacco cessation coaching sessions: An additional 300 points (Completion of this program is required to remove the tobacco surcharge.)
